WAIVER & CONSENT

ADVANTAGE FOOTBALL CLUB ACADEMY (AFCA)

This Waiver & Consent applies to every player registered for an AFCA program. Because most participants are minors, a parent or legal guardian must read and agree to this page on the player’s behalf when booking a session. By completing a booking, the parent or guardian confirms they have read, understood, and agree to the terms below.

ASSUMPTION OF RISK

Football/soccer training involves physical activity, close contact with other players, and use of equipment such as balls and cones. As with any athletic activity, there is an inherent risk of injury, including but not limited to sprains, strains, collisions, falls, and, in rare cases, more serious injury. AFCA coaches take reasonable care to run safe, age-appropriate sessions, but cannot eliminate all risk of injury associated with athletic training.

By registering a player for an AFCA program, the parent or guardian acknowledges this risk, confirms the player is physically able to participate, and agrees to hold AFCA, its coaches, and staff harmless from ordinary risks associated with athletic training, to the fullest extent permitted by Washington law.

MEDICAL AUTHORIZATION & EMERGENCY CARE

The parent or guardian is responsible for disclosing any medical conditions, allergies, or physical limitations that may affect the player’s safe participation, using the notes field at booking or by contacting us directly.

In the event of an injury or medical emergency during a session, AFCA staff will attempt to contact the parent or guardian using the phone number provided at booking. If a parent or guardian cannot be reached promptly and a coach reasonably believes emergency care is needed, the parent or guardian authorizes AFCA staff to call 911 or seek appropriate emergency medical treatment on the player’s behalf. AFCA is not responsible for the cost of any medical treatment received.

PHOTO & VIDEO CONSENT

From time to time, AFCA may take photos or videos of players during training sessions for use on our website, social media, or other marketing materials. By registering, the parent or guardian consents to this use unless they notify AFCA in writing (by emailing info@advantagefca.com) that they do not wish their player to be photographed or filmed. Players whose parents opt out will be excluded from promotional photos and videos to the extent reasonably practical.

CODE OF CONDUCT

We ask all players, parents, and guardians to treat AFCA staff, other players, and families with respect. AFCA reserves the right to remove any participant from a program for conduct that is unsafe, disruptive, or disrespectful, without refund of remaining sessions in cases of serious misconduct. ACKNOWLEDGMENT

By booking a session with AFCA, the parent or legal guardian confirms they are at least 18 years old, have the legal authority to consent on the player’s behalf, have read this Waiver & Consent in full, and agree to its terms along with our Terms and Conditions, Privacy Policy, and Refund & Cancellation Policy.
